How to submit a TEACH Grant Requirements Certification Form
Student Forms allows you to submit documents electronically directly to CSUF.Ìý The link is below after the instructions.
Please review the following before continuing to StudentForms:
1. ÌýYou will need to register with your student information before requesting a TEACH Grant Requirements Certification Form. Once you are logged on, you may request the form by selecting the "Manage Requests" button, and then selecting TEACH Grant Requirements Certification.

2. When requesting the TEACH Grant Requirements Certification , you will need to enter a brief explanation for the request.Ìý ÌýHere is an example:
